Assessment Task Instructions:
This scenario continues from the one provided in Assignment 1. It is recommended that you make a connection to your Assignment 1.
On your second day at work this week, your manager has once again assigned four-year-old Joe to your care. Following the handover, you conduct your initial observations and record the following: respiratory rate of 42 breaths per minute, temperature of 39.5°C, oxygen saturation at 90%, heart rate of 148 beats per minute, blood pressure of 105/70 mmHg, and a capillary refill time of 2 to 3 seconds. You observe that Joe is occasionally crying steadily and appears more restless than the previous day. He has visible nasal flaring, wheezing and prefers to remain sitting upright on the bed. His mother, Molly, expresses concern that Joe has a reduced appetite and is struggling to eat. Molly also mentioned that Joe is missing his seven-year-old twin siblings, Maria and Akim.
Based on the scenario above
- Explain the nursing assessments you will undertake to identify potential and actual nursing problems in the scenario.
- Describe the nursing management of Joe’s condition.
- Incorporate the child and family-centred approach in your care.
General tips on content
- Do not waste valuable time repeating the content of the clinical situation.
- Please do not waste time describing the tools/frameworks, rather apply them.
- Consider that your audience (Registered Nurses in the paediatric emergency department) know nothing about your task nor assume that they know what you mean consider refining and explaining in a way they can understand you.
