- Based on our incident report date between 2023 to 2024, we have 4 retained foreign objects (RFO), 49 mislabeled specimen, 10 lost specimen, 384 instrument equipment issues, and 220 surgical count issues. A post procedure debrief would serve as an opportunity to identify the near misses, to communicate concerns encountered during the procedure, to capture potential patient harm or address patient care issue, to reduce waste of operational resources, and to align with our policy.
- The benefits for doing a debrief is error prevention, maximized intended outcome, allows exchange of valuable feedback among team members and promotes open communication. To reduce variation and increase stability in process.
